About the Job

Objective/Overview
This position contributes to the efficiency and effectiveness of the clinic's services in providing patient care and care coordination for the Mobile Health Care, Psychiatry Department and/or Medical Department. This position reports to the Registered Nurse (RN) Supervisor.

Essential Functions

All patient Care & Care Coordination (40%)

  • Administer care/treatment and care coordination to patients as ordered by medical and mental health providers.
  • Participate and work together with nursing and multidisciplinary team members to ensure that patients receive whole-person care.
  • Conduct patient and family education related to preventive care, mental health care, chronic care and condition management as appropriate; connect patients to resources to meet their psychological needs and aid in their recovery.
  • Manage a panel of patients, many with multiple medical and mental health diagnoses, and other patient populations as assigned.
  • Develop patient care plans based on risk and overall patient care complexity.
  • Work with care managers to implement the patient's care plan.
  • Communicate with providers, care coordination team members, and other support staff through EHR, in person, and other means.
  • Communicate significant changes in patient status to the provider and care team.
  • Facilitate medication refills and provide medication education for patients as needed.
  • Identify and connect patients to community resources that will assist them in achieving stability, sustained recovery, and improved health outcomes.
  • Follow-up with patients if they have not scheduled their appointments. Check lab/path results from Fairview and if needed call Fairview for results.
  • Follow-up with no-show patients that have not rescheduled.
  • Work with interpreters and/or PSRs to call and schedule non-English speaking patients accordingly.
  • Orient residents and nursing students as needed.
  • Provide walk-in and phone triage. Empower patients to utilize appropriate resource for level of urgency (primary care vs. urgent care vs. emergency department)
  • Respond to flags, Emergency Department/Inpatient Hospitalization summaries, refill requests.
  • Respond promptly to Codes and other emergencies in the clinic, and work with clinical team members to deliver emergent care to patients.

SUBSTANCE USE DISORDERS CARE COORDINATION & MANAGEMENT (50%)

  • Provide Care Coordination services for patients eligible for the Suboxone program and for restricted patients.
  • Identify patients for the Suboxone program.
  • Thorough medical assessment and documentation of patient's signs and symptoms.
  • Receive and transcribe physician orders to client charts.
  • Manage dosage changes with providers.
  • Administer and dispense opioid replacement medicine as ordered (Suboxone).
  • Accurately and appropriately document client behaviors and self-reports.
  • Assist with client intake process (e.g. Enrollment paperwork, screening client appropriateness).
  • Provide patient education (e.g. risks and benefits of Suboxone, symptom management, understanding pain).
  • Provide education on relapse, addiction, and overdose.
  • Facilitate external referrals for medical needs.
  • Monitor lab tests as required (e.g. urine toxicity, blood tests).
  • Provide panel management, maintain contact requirements for best practices.
  • Safely dispose of unused medications.
  • Communicate changes to the Care Team.
  • Teaming with Suboxone providers, primary care providers, primary physiatrist providers, nurses, therapists, pharmacists, and other internal and external supports on patient's Care Team.
  • In conjunction with the provider, develop care plans for restricted patients, provide care coordination and manage restricted patients by communicating with providers, pharmacies, patients, etc.

Other Duties as Assigned (10%)

  • Attend all-staff meetings, weekly care coordination meetings, weekly nursing meetings, and daily huddles.
  • Other duties as assigned.

This position requires a criminal background check and negative TB test

Qualifications

Required Qualifications

  • Graduation from an accredited school of nursing. BA/BS degree or a combination of related education and work experience to equal at least four years.
  • Current licensure as a Registered Nurse in the State of Minnesota.
  • CPR certified.
  • Experience in managing clinical care using an electronic health record.

Preferred Qualifications

  • Two to three years of experience in providing care coordination or skilled nursing.
  • Experience working in a chemical dependency, addiction or mental health clinic.
  • Knowledge of community resources and/or social service agencies.
  • Proficient in one of CUHCC's dominant languages: Spanish, Somali, Vietnamese, Hmong, or Laotian.
  • Experience or certification in mental health nursing.
  • Experience in providing care coordination to a complex, diverse patient population.

Required Skills

  • Nursing assessment aptitude and problem-solving skills
  • Strong time management and organizational skills
  • Knowledge of regulations, policies, and procedures for nursing in the primary care setting.
  • Proficiency in utilizing an electronic medical record, registries or databases to monitor patient outcomes. Proficiency in electronic tools with the EMR to communicate with patients and clinical team members.
  • Must be able to lift 30 lbs.
  • Must be able to work in a fast-paced, rapidly changing environment that requires sitting, standing, and walking, with frequently changing priorities. Must be able to be flexible and multitask.
  • High-level communications skills to work with patients, staff, providers, and outside agencies.
  • Excellent customer service skills.
  • Willingness and skill in working with patients and staff from diverse cultures, languages, and backgrounds.
  • Capacity to work on a team and make positive contributions to the team.
