Position: Care Coordinator MSW
Shift: Per Diem, Varied Shifts. Weekend and Weekday availability needed.

Experience:
1. At least 5 years of recent clinical experience in a physician's office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience.
2. Leadership experience a plus.

Education:
Master's Degree in Social Work from CSWE (Council on Social Work Education) accredited program.

Licensures/Certifications:
Licensed Social Worker strongly preferred.
